Description
Mission
The mission of Transition Projects is to serve people's basic needs as they transition from homelessness to housing.
Strategic Vision
Transition Projects will be a recognized leader in helping people end their homelessness. We will achieve that by offering the most accessible programs for homeless persons in the region-with low barriers for entry and high expectations for quality, and by offering significant and meaningful ways for the public to engage in solving people's homelessness
Adopted June 21, 2008 in the agency's 3-year strategic plan
Values
Our core values are:
- Compassion/Caring. We care about our community, especially those individuals who are homeless or at risk of homelessness.
- Acceptance/Respect for Others. We accept people as they are and respect their differences as well as their shared characteristics and needs.
- Responsibility/Awareness. We have a responsibility to know what is happening in our community and how it affects individual members.
- Action for Change/Service. We will act to make our community a better place for homeless and at-risk individuals.
- All Basic Needs Met. We believe in social equity--that everyone, regardless of individual circumstances, should have basic needs for food, shelter and safety met.
Programs
ransition Projects provides people with the resources and tools they need to end their homelessness and maintain housing. Helping people meet their basic needs-shelter, clothing, food, cleanliness-is the bedrock of what we do. Once someone has met their basic needs, they can build the skills they need to transition to housing-goal setting, job skills, life skills, overcoming barriers to housing and income.
Our programs include:
Barbara Maher Building - Thirty-two units of supportive housing for women in recovery.
The Clark Center - A 90-bed shelter for men that provides the supports needed to move out of homelessness. The Clark Center features an on-site computer lab, mental health counseling, life skills, and case management. Stays are up to four months.
The Clark Center Annex - Twenty-two units of permanent supportive housing for men.
The Community Service Center - The front door to the agency, the Community Service Center provides showers, clothing, access to shelter waiting lists, information and referral, Community Voice Mail, mail, a message service, identification assistance, and housing assistance to homeless persons.
The Glisan Center - A 90-bed shelter for men, featuring: case management, life skills, mental health counseling, and a computer lab. Stays are up to four months.
Housing Outreach - Providing housing assistance, supportive services, a special outreach program for homeless women, and services to help people maintain their housing. Housing Outreach also provides eviction prevention, rental assistance, and case management for people on our shelter waiting lists.
Jean's Place - A 55-bed program for women. Jean's Place was the first program of its kind for women in Portland. It features an education and employment center, mental health counseling, case management, and life skills. The program includes the opportunity to lease a room at Jean's Place while rebuilding rental histories. Stays are up to eleven months.
