Resource Coordination

This free service is provided to Traumatic Brain Injury (TBI) survivors who are in need of brief case management support to connect them with community resources and services to assist in their continued recovery after a brain injury. The pilot project has Resource Coordinators working in four areas of the state including: Vancouver (Clark County) , Tacoma (Pierce County), Seattle (King County) , and Spokane (Spokane County). The goal of the service is to improve the quality of life for persons with Traumatic Brain Injury by assisting survivors and their families with "filling in the gaps" in their services.

Who is eligible for Resource Coordination services?

1. TBI survivors who live in Washington State.

2. TBI survivors with a brain injury in the last five years.

3. TBI survivors willing to share their health information with a Resource Coordinator to complete an assessment.

What do Resource Coordinators do?

• Work with TBI survivors and their families to assess their current needs and resources.
• Develop a comprehensive person-centered plan of care.
• Coordinate resources, services, and referrals for client advocacy.
